Where is the helm of hirol defence

Those " broken" items 4 I believe are a separate issue, these Hirol's items seem to be spread throughout the dungeon - I think I got 2 of them from various puzzles like the colour matching runestones and one from a chest in a room that appeared to be a storage area.
